Little Falls, NY – The Little Falls Community Outreach organization will host Live Well and Plan for the Future on Saturday, September 29, 2018 from 9:00 a.m. to 1:00 p.m., at 16 Jackson Street, Little Falls. The event is free and open to the public and will include numerous resources regarding healthcare planning and end of life wishes.

  • 11:15 a.m. – 11:45 a.m. – presenter, Shannon Cayea, Executive Director of New Hartford Palliative Care and Hospice, on Having Difficult Conversations regarding healthcare, end of life wishes and finances

*Resources in the Community forum will include presentations by Rae Raffle-Maxson, Program Director, Catholic Charities HICAP Program; April Savage, RN, Clinical Director, At Home Care; and Jennifer Miller, Administrator, Valley Residential Services, who will present on the continuum of care services provided by Valley Health Services and Valley Residential Services.
